Confirmation Sponsor Form
This form must be completed by those who wish to be sponsors for a confirmation candidate. Forms are to be submitted to the parish at which the sponsor is registered for verification. After the sponsor’s pastor has completed and signed the form, they must then be given either to the candidate’s religion teacher or the Religious Education Office in the Parish Administration Building.
