2024-25 School Year Physical Exam Requirements

Physical Examination Form, or your physician may use their own form.

All new students and incoming preschool, kindergarten, 3rd, and 6th graders are required to have a physical that is dated between 8/19/23 to present.  All physicals must be turned in before July 1, 2024. If the physical cannot be done before then, please contact the health room at x299 or email nurse@stjoecot.org.

Incoming 8th graders are required to show documentation of a T-dap and Meningococcal vaccination to be able to attend classes for their 8th grade year. This is mandatory prior to the start of school August 19, 2024. If no physical is received, then your student will be unable to attend on the first day of school.
