School Year Physical Exam Requirements

A completed Physical Examination Form is required for all new students and all incoming preschool, kindergarten, 3rd grade, and 6th grade students. Your physician may also submit their own physical form in place of the school form.

Physical forms must:

• Be submitted by July 1 for the upcoming school year
• Be dated within one year of the start of the school year

These physical exam records are mandatory for school attendance. Students without the required documentation on file will not be permitted to attend school on the first day.

If your child’s physical cannot be completed before July 1, please contact the Health Room at extension 306 or email nurse@stjoecot.org.

Incoming 8th Grade Immunization Requirements

All incoming 8th grade students must provide documentation of the required Tdap and Meningococcal vaccinations before the start of the school year in August.

These immunization records are mandatory for school attendance. Students without the required documentation on file will not be permitted to attend school on the first day.
